Chapter 7 Checklist The following documents are needed prior to filing your Chapter 7 case: - Copies of all pay statements received during the past six (6) months. After submitting the initial statements, please continue to provide copies of all statements received thereafter up until the date your case is filed with the court. If you are self-employed, or otherwise do not receive pay statements, please provide copies of other documentation establishing your income.
- Copy of your Federal Income Tax Return. All federal and state tax filings must be current.
- A Certificate of Credit Counseling for each debtor from an approved credit counseling agency. The counseling can usually be done on-line or by telephone. Here is a list of approved agencies www.utb.uscourts.gov under the "Credit Counseling/Debtor Education" box).
- Completed Creditor Worksheets.
- Completed Financial Worksheet.
